Isn’t 4(c) supposed to be the issuing authority instead of your residence? Or is it a Belgian thing because your government is confusingly complex?
All the licences I’ve had in Europe were always just listing the issuing authority of the country in question. It would have been a bit painful to get a new licence every time I moved.

You can also change your reporting of eye color too. (possibly even height) I know I did when they darkened
