Metaphor for what? The Lady is felating a guy seconds after entering the train, can’t be more on the nose than that.

What’s the joke here?
I’d say it’s more of a darkly ironic situation than a joke, but the humor comes from the juxtaposition of this apparently mutual and heartfelt expression of romantic fidelity against its unrealistically immediate violation by one party
