IIRC the context of the picture is that the pig was a show stud in a paddock at some fair, and that their balls just kind of do that when they go without spilling their vital essences for long enough. The rest of the pig is lean and fairly small, so growth hormones don’t make sense; if anything those should cause the opposite size distribution, with lots of muscle but atrophied balls.
